If you or someone you know has Diabetes, you can fill out a survey HERE and then receive a One Touch Glucose meter by mail. You even get to choose the color! The form is simple enough to fill out. Once you submit you will see Thank you! Your meter will be mailed to you shortly. That's it. It will continue on for other offers you can complete, but you don't have to complete those to get your meter at no cost.
